In order to carry out our mission, Palmetto Community Care uses a medical case management model for working with those we serve. Case management is a process by which staff works with clients to develop individualized service plans for how our clients will manage multiple aspects of HIV. Whether that be mental well-being, financial or medically related. The goal is for all clients to become self-sufficient, live with dignity and ensure they have a good support system.

Each client has unique needs that are addressed during the assessment interview.
Educate
Clients are educated about HIV, risk management, medical compliance.
Support
Case management staff uses supportive services to assist clients with their service plans.
Collaborate
A key part of our process is mutual collaboration so that clients can face any challenges together as a team.
