Living with HIV means managing a lot — medications, appointments, insurance, lab work, housing, finances, and mental health. It’s a lot for anyone to keep track of, especially when you’re also just trying to live your life.
That’s what case management is for. Our case managers work alongside you — not above you — to help you get what you need, stay on track with your care, and remove barriers to your care.
What Case Management Covers
Case management at PCC is built around your whole situation, not just your diagnosis. Depending on what you need, your case manager can help with:
Medical care coordination
- Connecting you with HIV medical care at Truesdale Medical Center or another provider
- Coordinating between your doctors, specialists, and care team
- Helping you stay on top of lab work, viral load monitoring, and follow-up appointments
- Supporting medication adherence so your treatment plan actually works
Insurance and financial assistance
- Enrolling you in health insurance or Medicaid if you’re uninsured
- Navigating Ryan White Program funding and the AIDS Drug Assistance Program (ADAP)
- Applying for medication assistance programs to reduce or eliminate out-of-pocket costs
- Help with benefits paperwork, appeals, and re-enrollment
Housing and basic needs
- Connecting eligible clients with emergency housing assistance and rental support
- Referrals to community housing programs and resources
- Help accessing food assistance, transportation, and other everyday needs
Goal-setting and planning
- Referrals to mental health and substance use counseling
- Connection to PCC’s support groups
- Peer support from staff who understand what you’re going through
Mental health and support services
- Working with you to build a personal service plan that reflects your priorities
- Tracking your progress and adjusting your plan as your situation changes
- Helping you build toward stability and long-term self-sufficiency on your own terms
Who Qualifies
Case management services at PCC are available to people living with HIV in Berkeley, Charleston, and Dorchester counties. You do not need to be a current PCC client to get started — if you’re living with HIV in the Lowcountry and need support, we want to hear from you.

Our Process

Assess
Every person’s situation is different. When you first connect with a case manager, we take the time to understand yours — your health, your living situation, your goals, and what’s getting in the way. There’s no judgment, just a real conversation

Educate
We make sure you have the information you need to make confident decisions about your health. That includes understanding your HIV status and treatment options, how your medications work, and what resources are available to you.

Support
Your case manager works with you to put a plan in place — and then follows through. Whether that means making calls on your behalf, helping you navigate a system that isn’t always easy to navigate, or simply being someone you can check in with, we’re here.

Collaborate
Your case manager doesn’t work in isolation. They coordinate with your medical providers, Truesdale Medical Center, and other services at PCC to make sure everyone on your care team is working together. You’re at the center of it.
