Medical Case Manager

The Medical Case Manager will be responsible for providing comprehensive case management services to persons living with HIV/AIDS within the boundaries of Charleston, Berkeley and Dorchester counties. The Medical Case Manager has the responsibility for securing appropriate and effective services to meet determined needs and to work within the SC Ryan White Part B Medical Case Management guidelines.
Social Media Ambassador

Our Social Media Ambassadors, better known as Palmetto Care Partners, is an online community of supporters who mix our messages onto their social media platforms. These ambassadors are critical to helping PCC expand our awareness of HIV, break down social stigma and help spread our message of compassion and care for those not only living with HIV but those that are at risk.
